Welcome to Acacia Adult Day Services
Acacia Adult Day Services provides community-based services that serve both the medical and social needs of individuals who are living at home, but need daytime assistance for their health and safety.
We serve individuals experiencing physical disabilities, cognitive difficulties, chronic health conditions, or life-long physical impairments. We emphasize services that enhance daily life for both the participant and the caregiver and stress a strong partnership and care coordination between the participant, the caregiver, and a primary care physician supporting the “whole person” for health and wellness.
Program and Services Options
Acacia Adult Day programs provide your loved ones with the opportunity to interact with their peers in an inviting community setting. The following programs are offered Monday – Friday based on individual need. All programs include: supervision, transportation to and from the center, a light breakfast, lunch, and snack, therapeutic activities, case management and social work services. Caregiver support groups are available in three languages at Acacia and other locations.
Adult Day Program – Offered to vulnerable adults, persons diagnosed with Alzheimer’s disease or related dementias and other disabilities. The program focuses on promoting health, socialization with peers, well-being and independence through a variety of small and large group therapeutic activities. Activities are designed to stimulate senses, wellness and memory.
Adult Day Health Care and Community Based Adult Services (CBAS) – Offered to individuals in need of supervised medical care. These programs provide a multi-disciplinary assessment, an individualized plan of care, nursing care including monitoring of vital signs and medication management, physical, occupational or speech therapy, personal care services, dietary counseling, and psychological counseling.
