How Do I Enroll?

Enrolling is easy… contact our social workers so they may help you assess whether Acacia offers programs that will help you and your loved ones. Whether or not we are the right answer for you, our social workers can help you move in the right direction towards getting help!

Step 1: The Call- your telephone call to our social workers will start the enrollment process. Call 714-530-1566 or email jbrooks@acacia-services.org to contact our social workers;
Step 2: We will ask you to visit- our social workers will ask that you come visit the center;
Step 3: Obtain the health record- You will be asked to request and provide us with a health record from the prospective participant’s physician;
Step 4: We'll visit you at home- Our social worker will schedule a home visit to find out more about how we can help you and your loved one;
Step 5: Review- Our social workers gather all the information obtained from the meetings with the family and from the physician's records, and present a complete profile to our team of health care professionals to review the case. The team then determines whether we are the program best equipped to help you!
Step 6: Start the program- if the team agrees that our program is appropriate for your loved one, he/she will be invited to join!

If the team concludes that the services we provide are not appropriate to address your needs, our social workers will refer you to other agencies that may be able to help you.
