How to Enroll
We make it easy for you to enroll your loved one at Acacia. If you are wondering whether Acacia’s programs are right for your loved one, contact our social workers (714-530-1566) so they can help understand what would be best for you. Whether or not Acacia is the right answer for you, we can help point you in the right direction.
Step 1: The Call
Start the enrollment process by contacting one of Acacia’s social workers.

Step 2: The Visit
A social worker will ask that you visit Acacia, so you can get a feel for our environment and what services we provide to participants. During your visit, we will give you a tour and answer any questions.
Step 3: Health Records
You will be asked to provide a health record from the prospective participant’s physician.
Step 4: Home Visit
One of our social workers will schedule a visit to your home, so that we can understand more about how we can help you and your loved one.
Step 5: Review
Our social worker will gather all the information from your home visit and health records, and present a complete profile to our team of health care professionals to review the case. Then, the team determines whether Acacia is the program best equipped to help you.
Step 6: Program Start
If the team agrees that our program is appropriate for your loved one, he/she will be invited to join Acacia as a participant.
If the team concludes that the services we provide are not appropriate in addressing your needs, our social workers will refer you to other agencies that may be able to help you.
